By your electronic signature of this form, you authorize charges to your credit card through Authorize.net via Shift Media Group, LLC for services rendered. These charges will appear on your bank/credit card statement as Shift Media Group, LLC. You have the right to request a paper copy of this document.

I authorize Shift Media Group, LLC to charge my credit card through Authorize.net.
I understand that this authorization will remain in effect until it is cancelled in writing and properly disputed, and I agree to notify Shift Media Group, LLC in writing of any changes in my account information or termination of this authorization.

I certify that I am an authorized user of this credit card and will not dispute these scheduled transactions with my bank or credit card company as long as the transactions correspond to the terms indicated in this authorization form. I acknowledge that credit card transactions could be linked to sensitive booking reservations which could potential not be rescheduled.

For bookings and reservation purposes:
I understand that a 7 day cancellation policy is strictly enforced for all day charters, and I will not receive a refund nor a transfer if I fail to comply. For Term charters, cancellations made 30 days or more in advance of the charter date, will receive a 100% refund or may reschedule to a different date/time, and no penalties will apply. Cancellations made within 14 days will incur a 50% fee. Cancellations made within 7 days to the charter will not receive a refund nor a transfer, unless absolved by force majeure.

Captain and client will conduct a walkthrough of the main areas of the vessel and safety rules prior to charter. Client understands that any damages created by client or his/her group will result on incidental chargers to the credit card attached.

You acknowledge that some third-party providers offering certain services and/or activities may require you to sign their liability waiver prior to participating in the service and/or activity they offer. You understand that any violation of any such supplier’s rules and restrictions may result in cancellation of your reservation(s), in your being denied access to the applicable charter or travel services, in your forfeiting any monies paid for such reservation(s), and/or in our debiting your account for any costs we incur as a result of such violation.
